Our PCN has recruited a specialist mental health practitioner. This role works across all practices in the network, and provides a bridge between primary care and specialist mental health providers. They can accept referrals from GPs for patient requiring help to access mental health services, and help patients transition back to the care of their GP when leaving hospital.

You can self refer into the Mental Health Practitioner through your local practice by completing a request online through your local practice website.
We also work with Hillingdon MIND who offer a range of local services including an adult recovery and wellbeing service, a mental health carer programme and free counselling. For more information on their services please go to their website.
