Clinical Pharmacists

Clinical pharmacists help to review and manage medicines by consulting with and treating patients directly. This could be over the telephone, or if needed face to face in surgery. They also help to manage long-term conditions, provide advice for those on multiple medicines and enable better access to health checks.

Patients may be contacted by a clinical pharmacist if they have long-term conditions like asthma, type 2 diabetes, arthritis or high blood pressure to make sure they are on the most appropriate medication and that it is working for the patient and they are getting the most out of their medications. They may also help by offering advice on lifestyle change to help manage the condition and to help individuals stay well.
